Healthcare Professional Profile
This is the standard template HealthcareApex uses for healthcare professional listings. It documents the fields shown on a verified profile so employers, recruiters, and patients know what to expect — and what we do not publish.
Permission-based listings only. HealthcareApex does not create profile pages for private individuals from inferred, scraped, or unverified data. Profiles are published only when supported by an active state license, NPI record, employer confirmation, or a direct submission from the professional.
Fields included in every profile
Each field is sourced from a public registry, employer disclosure, or the professional's own submission. Unverified fields are labeled.
Full name
Legal first and last name as published on the professional's active state license. We do not publish profiles for private individuals without verified, permission-based, publicly available data.
Job title
Current professional role, such as Registered Nurse, Nurse Practitioner, Training Coordinator, Caregiver, or Allied Health Therapist.
License type
Credential abbreviation (RN, LPN, NP, APRN, LCSW, OT, PT, RT, BCBA) tied to an active state-issued license.
Specialty
Primary clinical focus area: ICU, ER, pediatrics, oncology, home health, hospice, behavioral health, geriatrics, telehealth, or training and education.
City and state
Service area or workplace city and state. We list metro areas rather than home addresses.
Employer or workplace
Listed only when the employer is a publicly disclosed organization or the professional has explicitly submitted it for publication.
Years of experience
Self-reported or publicly documented years working in the listed role or specialty.
Certifications
Specialty board certifications, continuing education credentials, and recognized training (CCRN, CEN, CHPN, BCBA, CCM, BLS, ACLS, PALS).
Services offered
Types of clinical, training, or care services the professional provides, such as private duty care, telehealth visits, staff education, or care coordination.
Contact preference
How the professional accepts inquiries: secure message through HealthcareApex, employer-provided phone line, or workplace email.
Verification status
Indicates whether the listing has been confirmed against a state board license lookup, NPI registry, or employer confirmation. Unverified profiles are clearly labeled.

Profile publishing and verification
Do you publish profiles for individual healthcare workers?+
Only when we have verified, permission-based, publicly available data. We do not create profile pages for private individuals from inferred or scraped data. Professionals can claim or submit their own listing through HealthcareApex.
How is license information verified?+
Listed credentials are cross-checked against the appropriate state board (for example the Florida Board of Nursing, California BRN, or Washington DOH) or against Nursys for multistate license holders. Verification status is shown on every profile.
Can I request removal or correction of my profile?+
Yes. Any healthcare professional listed on HealthcareApex may request edits, corrections, or removal of their profile at any time by contacting our support team.
Are these profiles a substitute for hiring due diligence?+
No. HealthcareApex profiles are informational. Employers, families, and patients should perform independent license, certification, and reference verification before engaging any healthcare professional.
